A. No, you would only have to check for counter once for an automatic ability. "Deal 1 damage to your opponent" chosen for the resolution of this card's abilities once the condition "when this card attacks" is met are just actions for that effect. Even if you would use it 3 times, it would take place 3 times during the reslution of that effect, so there is no counter for each of those actions.
